医生或医师(英语:physician[a]、medical doctor、doctor),中国古代称大夫或郎中,是从事医学业务的专业人士,经由研习、诊断、预后、治疗各种疾病、创伤、身心损伤,以促进、维持、恢复健康。医师可能会将其执业重点放在特定的疾病类别、病患类型、治疗方法上,即专科医师,或可能负责为个人、家庭、社区提供持续性和全面性的医疗保健服务,即一般科医师[1]。“医生”的“生”指先生,为敬称。
中国三国东吴名医董奉晚年为百姓治病不收诊费和药钱,但要求病人痊愈后必须到山上种植杏树,名为“康乐杏”,后来“杏林”用于指“医界”[2]。
人力短缺
许多发展中国家有医生人力不足的问题。2015年,美国医学院协会提醒美国在2025年可能面临高达9万名医生的人力短缺。
社会角色和世界观
生物医学
受西方文化影响,近几个世纪以来医学越来越走向科学的还原论和唯物主义,医疗人类学家通常称之为生物医学[3],这种发展型态在现代工业化的世界占主导地位。生物医学“以文化上独特方式系统性的阐述人体和疾病”[4],是医学生学习的世界观。依此常规,医学模型是培训所有医生的完整程序[5],包括心理态度。
医生自身健康
有些评论员认为医生有责任在健康事务上为民众树立榜样,例如不吸烟[6]。确实在多数西方国家医生相对较少吸烟,他们的专业知识的确对其健康和生活方式产生有益的影响。根据对男性医生的研究[7],医生的预期寿命(白人73岁,黑人69岁)比律师或许多其他受高等教育的专业人士略高。与一般民众相比,医生较为少见的死因包括呼吸系统疾病(包括肺炎、尘肺病、慢性阻塞性肺病,但不包括肺气肿和其他慢性呼吸道阻塞)、酒精相关疾病、乙状结肠直肠癌、肛门癌、细菌性疾病[7]。
医生容易受职业危害,有格言说“医生会成为最糟糕的病人”[b][6],主因是医生发现自己有症状时,会比一般的病患焦虑,医生会将之前在医学院学习的疾病套用在自己身上[8]并推想最糟糕的诊断或结果[9]。焦虑可能使之拒绝承认症状或其意义[10][11][12],导致延迟就医[13]。焦虑可能影响病史,使之忽视重要的部分以避免为其诊治的医生做出严重的结论或行动[14]。医生较其他人多见的死因包括医生自杀、自我伤害、药物相关致死、交通事故、脑血管疾病、冠状动脉疾病[15]。医生也容易职业过劳,其表现为长期的压力反应,特征包含对病患的照护品质变差、情绪疲惫、个人成就感下降。美国医疗照护暨品质研究所的研究报告显示时间压力是造成职业过劳的最大原因。美国医学会的调查显示超过一半的受访者认为“太多的官僚任务”是职业过劳的主要原因[16][17]。
教育与训练
世界各地的医学教育训练与职业途径都不同。
发达国家
在所有发达国家,医学入门课程属于第三期教育,于大学的医学院进行,按照不同的地区和大学,可能接在中等教育或本科教育之后。中等教育一般费时五至六年来完成。本科教育常是三至四年的理科课程,其后的医学课程通常费时四至五年[18],因此获得医学学位可能需花五至八年的时间,按照不同的地区和大学而定。
在入门训练完成之后,刚毕业的医生需要一至两年的时间在监督下行医,此时为实习医师[18]。
毕业生拥有大学给予的医学学位,使其有资格根据特定国家或地区(有时是多个国家或地区)的法律许可取得医师执照,但必须满足实习或其他额外条件。
规范
在大多数的司法管辖区,医生需要政府许可才能执业,除了是为了促进公共安全,也能保护政府支出,因为各国政府常提供医疗补贴。未经许可而行医者称为密医。
在某些司法管辖区,例如新加坡,即使医生仅有基本理学士学位(非医学士),也常会在信件或名片中使用“Dr”作为称呼。在德国等其他国家或地区,只有拥有学术医学士学位的医生才能自称为医生,另一方面,欧洲研究理事会判定德国医学士学位不符合哲学博士学位的国际标准[19][20]。
绩效和专业监督
医疗疏失、药物滥用、其他医生专业问题在全世界引起广泛关注[21],特别是2000年一项重要报告[22]发表后,开启了病患安全运动[23]。
